Ranolazine and Ivabradine - Their Current Use

چکیده
Chronic stable angina is a debilitating illness affecting millions worldwide. Considerable progress has been made over the last 30 years in the understanding of the pathogenesis of angina pectoris. Over the period, number of pharmacological measures such as nitrates, beta-Blockers, calcium channel blockers and revascularisation strategies such as Coronary Artery Bypass Grafting (CABG) and Coronary Angioplasty have been introduced but still many patients continue to experience Angina despite being on optimal medical therapy. In addition there are a number of patients particularly elderly patients who are unsuitable for revascularisation procedures or they do not tolerate higher doses of conventional anti-anginal drugs. In such cases a novel medical treatment with different mechanism of action would be particularly beneficial in relieving the symptoms. RANOLAZINE AND IVABRADINE THEIR CURRENT USE

New Developments in Anti-Anginal Therapy: Roles of Ivabradine, Allopurinol and of Agents Modifying Myocardial Metabolism
Over the last 20 years, it has emerged that, while surgical revascularisation of extensive ischaemic heart disease may have prognostic advantages, the main issues considered regarding individual management are usually those of symptomatic improvement only. The major impetus towards invasive intervention is therefore failure of prophylactic anti-anginal therapy. On the other hand, many patients,...
متن کاملThe challenges with chronic angina.
Should therapies to treat chronic stable angina reduce the risk of major cardiovascular events such as death and myocardial infarction? Although this may be a laudable target, the majority of treatments that are currently in use, such as nitrates, calcium-channel blockers, and betablockers, have not been proved to achieve this goal. Still, these agents have been recommended as first-line therap...
متن کاملThe role of optimal medical therapy in patients with refractory angina
The management of patients with refractory angina poses a major clinical challenge. Optimal medical therapy (OMT) is of paramount importance to improving quality of life by reducing (as much as possible) the number of angina attacks with a correspondent increase in exercise tolerance. For that, a judicious use of a combination of all agents currently approved in the management of patients with ...
متن کاملFacts and numbers on epidemiology and pharmacological treatment of heart failure with preserved ejection fraction
Heart failure with preserved ejection fraction (HFpEF) is a major and growing public health problem. Epidemiologic studies demonstrated that heart failure (HF) can be clinically diagnosed in patients with normal or preserved left ventricular ejection fraction. These patients are therefore termed as having HFpEF. In the past, this was often called diastolic HF. Because of the permanent increase ...
متن کاملRecurrent angina after coronary angioplasty: mechanisms, diagnostic and therapeutic options.
Recurrent angina in patients who underwent percutaneous coronary intervention is defined as recurrence of chest pain or chest discomfort. Careful assessment is recommended to differentiate between non-cardiac and cardiac causes. In the case of the latter, recurrent angina occurrence can be related to structural ('stretch pain', in-stent restenosis, in-stent thrombosis, incomplete revascularizat...
